Personal Wellness Journal Guidelines
The purpose of this activity is to teach students the principles of health promotion, self-care, change theory,
health education, health planning and evaluation.
COURSE OUTCOMES
This assignment enables the student to meet the following course outcomes:
CO2. Examine theories and concepts related to health promotion behaviors. (PO#1 & 2)
CO7. Identify health promotion strategies throughout the lifespan. (PO#1 & 2)
REQUIREMENTS AND GUIDELINES
• Identify priority areas to improve one’s own health and wellness (lifestyle).
• Write at least one wellness goal to implement during this session. Goal may be related to but not limited
to: physical activity, sleep and rest, recreation, nutrition, stress reduction, weight management, time
management, and/or relationships.
• Describe day-to-day activities, events, challenges, achievements, plans, and progress related to goal in
a weekly journal. Submit three journals on the timeline specified in the course calendar. The first two
submissions will be for review and comment. Evaluate progress toward the goal and discuss progress
or lack of progress, what strategies worked and did not work (formative evaluation). From week to week
adjust your plan to achieve your goal.
• Three journals, one each per week for three consecutive weeks:
o First two (2) journals will be submitted in the Dropbox before class on due dates provided in the
course calendar.
o Faculty will review and comment on the first and second journals (i.e. no points).
o Only final journal will be allocated assignment points; must have completed all journals on time in
order to get full points for the assignment.
• All information will be kept confidential and used solely for the purposes of this assignment.
• Faculty will debrief by having a discussion on what was learned as long as individuals are willing to
share. Also discuss how this activity will help the student provide nursing care.
POINTS FOR ENTIRE PROJECT: 100 POINTS
The final journal should be a review of all three weeks progress, measure achievement of the goal and
express future plans for promoting your health (summative evaluation).
