Suppose an organization you work or volunteer with (preferable related to healthcare) is conducting a program evaluation and has asked you to design a survey instrument that will help the organization understand it’s clients’ perspectives on the organization’s current strengths as well as opportunities for improvement. Design a questionnaire that uses a variety of different types of questions. Edit the draft survey instrument several times to be sure that the questions cover most important topics, the type of question is appropriate for each item, the response options are appropriate, the questions are worded clearly and unambiguously, the order of questions is suitable, and other potential problems have been resolved.
